Founded in 1997, CareLink is a management services organization providing services to mission-driven, post-acute and aging care providers. Comprised of 9 corporate members representing 25 providers through-out the state of Rhode Island. Our post-acute care network includes the full spectrum of care from adult day health and long-term care to senior housing and home health. We are a non-profit mission-based 501c3 organization.

~ We manage the complete network so your patients and residents successfully find the care they need at the right place and time. This includes adult day services, home care, assisted living, short- and long-term care, hospice care, outpatient, and independent living.

~ Our members meet specific quality and service standards and are supported by CareLink’s program development and improvement opportunities.

~ We don’t shy away from change; we embrace it. We constantly update and collect data for outcomes, quality, and results, demonstrating the system’s impact on quality of life.
